Program

The Mississauga Football Club takes pride in presenting its extensive and year-round Development Training program, tailored for soccer trainees of every skill level. This holistic initiative transcends seasonal limitations, ensuring a continuous and immersive soccer experience that goes beyond traditional boundaries.

Central to the success of our Development Training program are our esteemed coaches, possessing licenses from both the Canadian Soccer Association (CSA) and the Union of European Football Associations (UEFA). Their wealth of experience and commitment to excellence provides participants with a professional training environment, fostering a culture of growth and skill enhancement. Furthermore, our coaching team includes active professional players, imparting firsthand knowledge and insights that elevate the training sessions to a truly exceptional level.

Informed Consent and Acknowledgement

I hereby give my approval for my child’s participation in any and all activities prepared by Mississauga Football Club. In exchange for the acceptance of said child’s candidacy by Mississauga Football Club., I assume all risk and hazards incidental to the conduct of the activities, and release, absolve and hold harmless Mississauga Football Club, and all its respective officers, agents, and representatives from any and all liability for injuries to said child arising out of traveling to, participating in, or returning from Mississauga Football Club training.

In case of injury to said child, I hereby waive all claims against Mississauga Football Club; including all coaches and affiliates, all participants, sponsoring agencies, advertisers, and, if applicable, owners and lessors of premises used to conduct the event. There is a risk of being injured that is inherent in all sports activities, including soccer. Some of these injuries include, but are not limited to, the risk of fractures, paralysis, or death.

Medical Release and Authorization

As Parent and/or Guardian of the named athlete, I hereby authorize the diagnosis and treatment by a qualified and licensed medical professional, of the minor child, in the event of a medical emergency, which in the opinion of the attending medical professional, requires immediate attention to prevent further endangerment of the minor’s life, physical disfigurement, physical impairment, or other undue pain, suffering or discomfort, if delayed.

Permission is hereby granted to the attending physician to proceed with any medical or minor surgical treatment, x-ray examination and immunizations for the named athlete. In the event of an emergency arising out of serious illness, the need for major surgery, or significant accidental injury, I understand that every attempt will be made by the attending physician to contact me in the most expeditious way possible. This authorization is granted only after a reasonable effort has been made to reach me.

Permission is also granted to Mississauga Football Club and its affiliates including Directors, Coaches, and Team Parents to provide the needed emergency treatment prior to the child’s admission to the medical facility.

Release authorized on the dates and/or duration of the registered season and all future participated programs.

This release is authorized and executed of my own free will, with the sole purpose of authorizing medical treatment under emergency circumstances, for the protection of life and limb of the named minor child, in my absence.

Photography & Video Release

I hereby grant Mississauga Football Club, permission the right to photograph my child's image, likeness and sound of their voice as recorded on audio or video tape. I understand that my childs image may be edited, copied, exhibited, published or distributed on social media channels such as Instagram, Facebook, Twitter & the website website (www.fcmississauga.com)

Photographic, audio or video recordings may be used for the following purposes:

By signing this release, I understand this permission signifies that photographic or video recordings of my child may be electronically displayed via the Internet or in the public educational setting.

I will be consulted about the use of the photographs or video recording for any purpose other than those listed above.

This release applies to photographic, audio, or video recordings collected, video clips and images, online, and Social Media channels as part of the sessions listed on this document only. I agree and acknowledge that photography and/or video can be used even after my child has left the soccer program.

Additionally, I waive any right to royalties or other compensation arising or related to the use of my child's image or recording.

BEHAVIOUR EXPECTATION & SIGNING COMMITMENT

As a Mississauga Football Club competitive soccer player, you have the responsibility to behave in a way in which upholds the Core Values of the Club: Pride, Passion, Professionalism and Perseverance. As elite footballers you are role models. As a club, we want to be remembered for our exemplary conduct both on and off the field of play. The Club management takes player conduct very seriously and will take action in the event of any activity or incident which may be considered a breach of this code.

As such you must:

Give your all and make every effort to reach your potential.

Set a positive example to others.

Be on time or even better, be early.

Abide by the instruction of the Academy - Club staff.

Show respect and be polite with everyone you encounter.

Respect differences between people.

Be careful when using the internet, social media, or other online communication methods.

Not use or tolerate inappropriate language.

Be dressed appropriately and in accordance with the Academy rules.

Know and play within the Laws of the Game and spirit of fair play. Respecting game officials and coaches of the opposite team.

Eat and drink well.

Manage illnesses, injuries and rehabilitation in a professional manner.

Avoid any activity, which may bring you, others, the Club or the sport into disrepute.

Understand if practices or games are missed for other soccer programs not affiliated with Mississauga Football Club, you will be removed from the Club and No refunds or credits will be provided.
